You will work as part of a team that promotes social inclusion and access to services for individuals in recovery from mental health difficulties, focusing on their individual needs and wishes.
Responsibilities
* Provide support and time to an allocated group of service users to promote and maintain their recovery within the community.
* Assess, plan, promote and review individualised activity plans using team procedures.
* Assist care coordinators in developing holistic individualised care plans under the ICPA.
* Manage a caseload of up to 20 clients with minimal supervision.
* Work within Trust policies and procedures.
* Travel within the geographical work area effectively.
* Work weekends and evenings as appropriate to meet client needs.
* Partner with local providers to promote access to community activities and build sustainable relationships that support clients long term.
* Drive as required; a full and valid UK driving licence is essential and you will use your own vehicle on Trust business.
Qualifications & Requirements
* Enhanced Disclosure and Barring Service (DBS) check – cost covered by the Trust.
* Full and valid UK driving licence.
* Ability to travel within the designated work area.
* Willingness to work weekends and evenings as needed.
* Strong interpersonal and organisational skills.
* Capacity to work autonomously with minimal supervision.
* Note: Candidates requiring current Skilled Worker sponsorship to work in the UK are not eligible to apply for this position.
