We are looking for dynamic Assistant Practitioners to join the East Ross Discharge to Assess team. You will work with individuals in their own home, leading on non complex assessment on discharge from hospital and delivering targeted and intensive rehabilitation programmes to maximise patient independence.
This is an exciting opportunity to work within an established Integrated team in a community based setting focusing on facilitated discharge and prevention of admission.
You will be expected to manage a caseload, to provide assessment, rehabilitation, onward referral and discharge to a range of patients referred to the service. You will work within multi-professional teams to ensure co-ordinated care for patients.
Professional guidance and day to day management will be provided through an Advanced Practice Occupational Therapist. You will work on a shift pattern of 4 days on, 4 days off to ensure continuity of service for our patients.
Regular supervision is provided (including peer supervision opportunities) and opportunities for ongoing professional development are encouraged.
You will be expected to travel efficiently and effectively between various work locations and patient residences, so a full current driving licence and use of a vehicle is essential.
