Community Support Worker - Community Response Team (CRT)

An exciting opportunity has arisen for an enthusiastic Band 3 Community Support Worker to join our Community Response Team. If you are a carer, support worker, or someone interested in working for the NHS, we would love to hear from you!

Our Community Response Team covers South Warwickshire with bases at Stratford upon Avon and Leamington Spa. We work across the county supporting patients with identified health and social needs as a result of changes in their health condition. The service is committed to providing high quality care to prevent avoidable hospital admissions, enabling patients to remain independent in their own homes and facilitating safe discharge from hospital. The team works closely with social services, GPs, community nursing teams, patients, relatives and carers.

To be successful, you must hold a valid driving licence and have access to your own car.

The Community Response Team operates 08:30-21:30, seven days a week, including weekends and bank holidays. Shifts are allocated pro rata, offering a mix of early, late, and long days.


Main duties of the job

The Band 3 Clinical Support Worker will undertake high quality, caring, and compassionate personal care and associated duties under the direction, but not direct supervision, of registered staff as part of the multidisciplinary team.

As a Band 3 Clinical Support Worker (CSW) you make a valuable contribution to the delivery of high quality healthcare, supporting the registered professional in the safe, effective, compassionate patient care. This description is read in conjunction with the person specification.


Job responsibilities

Clinical Practice

• Accurately and promptly carry out instructions from the registered professional.

• Assist in supervising other healthcare and support staff as directed.

• Carry out duties per the approved core task list for Clinical Support Workers/Healthcare Assistants without direct supervision.

• Report to the registered professional any patient concern or observed changes.

• Deliver high standard individualised care under the direction of the registered professional.

• Report all care delivered to a registered professional.

• Accurately document all care, countersigned by a registered professional.

• Assist in preparing patients for treatment, investigations or procedures.

• Keep clinical area clean and tidy, maintaining a safe environment.

• Adhere to infection control procedures and understand cross‑infection.

• Chaperone or escort patients under the direction of the registered professional.

• Support relatives with compassion and empathy under guidance of a registered professional.

• Assist with the discharge process as directed.

Communications and Interpersonal Relationships

• Welcome patients, relatives, and visitors, providing assistance and information.

• Be aware of communication barriers and address them.

• Introduce yourself in line with Trust privacy and dignity promises.

• Answer the telephone politely and forward enquiries as necessary.

• Communicate effectively within multidisciplinary team and other departments.

• Contribute to reporting, handover, and documentation of patient care.

• Report incidents and accidents per Trust policy.

• Use the Trust information support system and other computer systems as required.

Team Working

• Safeguard patients’ property and promote a professional environment.

• Share responsibility for equipment and cleaning.

• Dispose of waste and soiled linen per Trust policy.

• Participate in audit under the registered professional.

• Clean and check beds, spaces, and lockers in patient areas after discharge.

• Store deliveries correctly and maintain stock levels.

• Support housekeeping duties and report cleaning needs.

Education, Training and Personal Development

• Attend mandatory training (infection control, resuscitation, manual handling).

• Complete the Trust’s Clinical Support Worker/Healthcare Assistant Induction programme.

• Participate in appraisals, personal reviews, and work to achieve objectives.

• Undertake training for development of the post and department relevance.

• Continue personal development in relation to objectives.

• Maintain up‑to‑date knowledge of policy changes relevant to the post.


Person Specification


Qualifications

* Obtained an educational level of GCSE level A - C in English and Maths or equivalent
* Minimum NVQ/Diploma Level 2, willing to work to Level 3 or equivalent higher apprenticeship in healthcare
* NVQ/Diploma level 3 or equivalent higher apprenticeship in healthcare


Experience

* Experience delivering care in a healthcare or people‑centred environment
* Experience working as part of a team


Skills

* Evidence of competence in care skills
* Good written, verbal and non‑verbal communication skills
* Able to present information written and orally in an accurate clear and logical manner
* Ability to organise and prioritise workload
* Ability to write accurate and legibly in patient notes
* Evidence of competence in relevant specialist skills/tasks


Personal Qualities

* Able to work as part of a team
* Compassionate and caring nature
* Reliable


Other

* Able to work flexibly over a 24 hour period
* Able to carry out moderate to intense physical effort throughout the working day and carry out concurrent activities
* Able, with appropriate support, to work in an emotionally demanding environment
