We are offering an exciting position for a full-time temporary Grade 10 Advanced Practitioner in Social Work to support the current Practice Managers, within the Macclesfield Adult Social Care Community teams. This is a post aimed at the Prevent, Reduce and Enable project and the base is at Macclesfield Town Hall and there is an expectation that the Advanced Practitioner will be available on site as needed, the successful applicant will report to and be supported by the Practice Manager.
The focus of this Advanced Practitioner role is to work with the dedicated OT, social care staff, Reablement service as well as the Community Development Officer and LAC to support new individuals on ensuring that a quick response is provided with the support Reablement and aids and equipment to prevent or reduce longer term needs. The Advanced Practitioner will oversee allocations and ensure that assessments are strengths-based and person-centred, whilst ensuring that interventions represent a proportionate response to presenting issues. The Advanced Practitioner will work closely with the Practice Manager and oversee any safeguarding concerns and check effective signposting to alternative services. They will also oversee carers assessments and must be able to handle the demands and pressures of the role. The Advanced Practitioner will give guidance to staff to support their decision-making and oversee their supervisions. The Advanced Practitioner would not be expected to deal with budgetary, staffing or team performance issues.
This is an exciting opportunity to develop leadership skills and trial a different role. The successful candidate will have relevant, up-to-date, experience of using an asset-based approach in the community, which promotes health and well-being and seeks to prevent, reduce or enable each individual that is presented for this pilot process.
What we are looking for:
* An experienced and enthusiastic Grade 9 Social Worker who has a good understanding of adult safeguarding concerns.
* A Social Worker with some experience of supervisions and have an understanding of the Prevent, Reduce and Enable agenda.
* The candidate should be self-motivated with excellent communication skills and have the ability to work within the Adult Community Teams and wider multi-disciplinary arena.
* Good organisational skills and time management is key to effectively supporting staff and the ability to feed back at regular meetings in a clear and concise way.
* The candidate should be able to provide a client centred approach to care and demonstrate sound knowledge through evidence-based assessments and management of support in a variety of situations.
* It will be essential for the post holder to work autonomously within the Council’s policies and professional practice, in addition to maintaining accurate and comprehensive client documentation.
The post holder will be based at Macclesfield Town Hall, but hybrid working is available and includes working at home on occasions and there may be a necessity to travel to other bases.
