We're looking for an experienced practitioner with a strong sense of social justice to join our Homeless Health Inclusion Team (HHIT). We deliver transformational models of holistic healthcare to people experiencing homelessness, the Gypsy and Traveller community, and other inclusion groups in Leeds through outreach and in-reach programmes in partnership with other services. We advocate for our patients, plan safe discharges, provide out-of-hospital care, and lead healthcare for patients in Transitional Housing Units and those placed in health-commissioned beds. We also outreach to the Gypsy and Traveller community, working with Leeds Gypsy and Traveller Exchange and facilitating health equity for their members. The HHIT team has links with street teams providing street outreach, signposting, and building relationships with those rough sleeping and new to Leeds. The team works with individuals suffering from homelessness, sofa surfing, vulnerably housed, victims of trafficking and exploitation, vulnerable women fleeing domestic violence, refugees, asylum seekers, failed asylum seekers, and those with no recourse to public funding.
You will manage a team of healthcare professionals and care navigators, hold a caseload, and work with patients on their health goals to meet health needs and reduce emergency hospital admissions.
Main duties of the job
Duties include:
* Delivering, leading, and managing the HHIT team.
* Developing new and innovative service initiatives.
* Ensuring clinical pathway delivery.
* Supporting the head of service in managing the service budget.
* Coordinating complex, safe discharge planning from hospital.
* Identifying and implementing appropriate interventions.
* Providing education to stakeholders.
* Engaging in multi-agency collaboration with all partners in Leeds working with marginalised groups.
* Representing the service at city-wide meetings.
The team is based at Burmantofts Health Centre, with outreach and in-reach activities across the city. The service operates Monday to Friday, 08:30 to 16:30.
Suitable candidates may include Community Nurses, Health Visitors, Occupational Therapists, or Mental Health Nurses. Experience working with inclusion health groups and in challenging environments is essential. Flexibility, resilience under pressure, and effective communication skills are crucial, especially when working with individuals who have experienced complex trauma. You should be able to work independently while maintaining a strong team and professional identity.
