What You Will Do:
Conduct person‑centred, strengths‑based assessments to promote independence and wellbeing.
Develop and implement tailored care plans in collaboration with the Social Services Team.
Maintain accurate and up‑to‑date electronic case records.
Work flexibly to meet service user needs while prioritising safety and quality of care.
Travel within the local area (car driver preferred) to complete community‑based visits.
About You:
Qualified Occupational Therapist.
HCPC registered, maintaining professional standards and compliance.
Experience within Adult Social Care settings.
Strong assessment, communication, and partnership‑working skills.
