Reablement & Bridging Senior Care Officer
We are seeking a dedicated and passionate Reablement & Bridging Senior Care Officer to join our dynamic team. Responsibilities
* Lead the assessment and development of personalized care plans for clients to support their reablement and bridging needs.
* Coordinate and oversee the delivery of care services, ensuring compliance with relevant regulations and quality standards.
* Supervise and mentor junior care staff, providing guidance, training, and support to enhance team performance.
* Collaborate with healthcare professionals and social workers to create a multidisciplinary approach to client care.
* Monitor and evaluate client progress, adjusting care plans as needed to ensure optimal outcomes.
* Act as the primary point of contact for client families, addressing concerns and providing updates on care progress.
* Maintain accurate records and documentation related to client care and team performance, ensuring confidentiality and compliance with data protection policies.
Requirements
* Proven experience in a senior care officer role or similar position within the health and social care sector.
* Strong understanding of reablement and bridging care principles, with the ability to apply them effectively.
* Excellent communication and interpersonal skills, with a compassionate approach to client care.
* Demonstrated leadership skills, with experience in supervising and mentoring staff.
* Relevant qualifications in social care, health, or a related field (e.g., NVQ Level 3 or equivalent).
* Ability to work collaboratively within a multidisciplinary team and engage effectively with clients and families.
* Strong organizational skills and the ability to manage multiple priorities in a fast-paced environment.
