Overview
We are looking for a clinician with a passion for older adult care to join the South and Dales OA CMHT. The role will primarily involve working into the Care Home project, independently carrying out light touch clinics with care homes, providing non-pharmacological advice and support, managing a complex caseload of patients in the care home setting, and carrying out assessment and intervention work alongside other clinical team members.
The post holder will also work into the duty rota, providing single point of access cover, and will have the opportunity to develop leadership skills as a link worker or champion (e.g. student placement link worker, medicines champion, clozapine link worker).
The team includes nurses, NMPs, occupational therapists, psychologists, nursing assistants, consultant psychiatrists, junior doctors, and administrators, all working together to provide high quality care.
Responsibilities
* Independently carry out light touch clinics with care homes.
* Provide non-pharmacological advice and support.
* Manage a complex caseload of patients within the care home setting.
* Carry out assessment and intervention work alongside other clinical members of the team.
* Provide single point of access cover on the duty rota.
* Develop leadership skills and act as a link worker or champion.
* Participate in multidisciplinary meetings, CST meetings and the Care Homes Project.
* Assist with supervisory duties for staff.
Qualifications & Skills
* Excellent communication skills and the ability to complete thorough assessments, including safety assessments.
* Experience producing person-centred care plans with a living well/recovery focus within the CPA framework.
* Strong teamwork and collaboration across disciplines.
* Experience working with older adults and caring for organic and functional illnesses.
* Ability to adapt to a challenging and varied caseload.
