We arelooking for a motivated and committed Clinical Pharmacist to join ourfriendly, forward-thinking practice team. Working across our two sites, youwill support patients and staff with medicines optimisation, medication reviewsand prescribing queries. You will work closely with GPs, other pharmacists,nurses and wider colleagues to deliver safe, evidence-based care and improvehealth outcomes for our diverse patient population.

Main duties of the job
* Conductstructured medication reviews, including for patients in high-risk groupssuch as frailty, polypharmacy, and high-risk medications.
* Managecomplex polypharmacy and reduce inappropriate prescribing where possible.
* Supportmanagement of chronic diseases through safe prescribing and monitoring.
* Managethe repeat prescription system, including reauthorisation, requestingblood tests, and responding to prescription queries.
* Completemedicines reconciliation following hospital discharge and transfers ofcare, liaising with patients and community pharmacists to ensurecontinuity of supply.
* Supportthe development and implementation of cost-effective prescribingstrategies.
* Provideleadership in medicines optimisation, medicine safety projects, audits andquality improvement initiatives (including QOF work).
* Implementa medication review system to ensure patients are recalled for monitoringand reviews throughout the year.
* Contributeto practice meetings, sharing expertise and supporting servicedevelopment.
* Referto senior GPs when appropriate and involve patients in shareddecision-making about their medicines.

Job responsibilities
* Runclinics to conduct medication reviews and resolve patient medicationqueries (mainly via telephone consultations, with a blend of remote andon-site working).
* Reconcilemedicines following hospital discharge or transfer of care.
* Managerepeat prescription requests and queries within scope of practice.
* Implementchanges in line with MHRA alerts, product withdrawals, shortages, andguidance.
* Signpostpatients to appropriate services or healthcare professionals wherenecessary.
* SupportQOF, IIF and quality improvement projects.
* Undertakeaudits and implement recommendations.
* Maintainaccurate clinical records, using templates and coding appropriately.
* Identifyclinical risks and contribute to developing safer, more effective systems.
* Maintainclinical competence through CPD and professional development.
Person Specification
Qualifications
* Registered with the General Pharmaceutical Council
* Independent Prescriber (or working towards this qualification)
* Strong therapeutic and clinical knowledge with understanding of evidence-based practice
* Experience of medication reviews and medicines optimisation
* Excellent communication, interpersonal and influencing skills
* Ability to work independently and as part of a team
* Good IT and record-keeping skills
* Commitment to professional development
* Experience of working in primary care or general practice
* Membership of PCPA or RPS
* Audit and quality improvement experience
