Job overview
We are looking for an experience Critical Care Nurse to join our Outreach team.
If successful, you will provide expert nursing advice, care and support to patients that are at risk of deterioration or are critically ill. This will also include their families and carers.
You will provide support until either the patient is transferred to the critical care unit, their condition has improved or continuing this level of care is agreed that it is not in the patient's best interest.
You will be pivotal to developing and monitoring standards within the Critical Care Outreach (CCOT) service, including supporting and educating more junior team members.
Using your experience and expertise you will also act as a specialist resource of up to date clinical information for the multi-disciplinary team (MDT).

Main duties of the job
* To act as a the lead member of the CCOT team providing early intervention. To act as a skilled member of the Critical Care Outreach team providing early intervention with appropriate assessment, observation and treatment of seriously ill patients in general ward areas i.e. level 1 and level 2 patients.
* Act as a source of expert advice and support to those caring for the deteriorating patient / critically ill patients and their families on the wards.
* Work as part of a multi-disciplinary critical care team providing as a point of contact for the critically ill patients. To make clinical judgements, work autonomously managing own workload, seeking advice and guidance from senior Critical Care nursing and medical staff as necessary.
Promote the nursing contribution to both the Directorate's and Trust's clinical governance plans.
Undertake projects, audit, and research to improve deteriorating patient/high dependency care skills trust wide.
Provide multi-disciplinary education in the deteriorating/high dependency care patient trust wide including the development of doctors in training and qualified nursing staff.
To undertake trust agreed advanced specialist clinical practice skills to meet patient needs.
