Central and South Primary Care Network is looking to recruit an experienced Social Prescribing Link Worker to join our multidisciplinary team.

This is an exciting opportunity to make a real difference to patients lives by helping them take greater control of their health and wellbeing. You will connect people to community-based support and services that address non-medical issues affecting their health, such as loneliness, housing, employment, debt, and lifestyle.

This role is funded through the Additional Roles Reimbursement Scheme (ARRS) and forms a vital part of our commitment to personalised care and improving population health outcomes across Knowsley.

Salary and Hours

* 20 hours per week, worked flexibly over 3 or 4 days


Main duties of the job

Main Duties of the Job

* Support patients referred by GPs and practice teams to identify personal goals and develop individualised wellbeing plans.
* Help people access appropriate community, voluntary, and social enterprise (VCSE) services.
* Work closely with GP practices, the wider PCN team, and local partners to ensure coordinated care.
* Maintain accurate records and contribute to practice systems to ensure continuity and evaluation of support provided.
* Build and maintain strong relationships with community organisations to expand local support options.
* Promote equality, inclusion, and health equity in all areas of work.

Job responsibilities

Key Responsibilities

* Provide personalised support to individuals, enabling them to identify their goals and co-produce a personalised plan to achieve these.
* Act as a key point of contact for patients, offering continuity and coordination of care.
* Support patients referred by GPs, practice staff, and other healthcare professionals to access non-clinical services and community resources.
* Work with individuals to build their confidence, resilience, and independence by identifying issues affecting their health and wellbeing.

2. Collaboration with General Practice

* Work closely with GPs and the wider multidisciplinary team to identify patients who would benefit from social prescribing interventions.
* Contribute to clinical meetings and case discussions to provide insights into non-clinical solutions for patients.
* Maintain accurate and timely records of referrals, interventions, and outcomes, ensuring integration with the practices clinical systems.

3. Community Engagement and Development

* Develop strong links with local voluntary, community, and social enterprise (VCSE) organisations to expand referral options and resources available to patients.
* Work collaboratively with local stakeholders to map and maintain an up-to-date directory of services.
* Identify gaps in service provision and work with the PCN to address these.

4. Holistic Care and Best Practice

* Apply a holistic approach to patient care, considering physical, emotional, and social needs in line with ARRS guidance and NHS Englands personalised care model.
* Promote health equity by supporting patients from diverse backgrounds, including those from vulnerable or marginalised groups.
* Adhere to relevant legislation, including data protection, safeguarding, and equality and diversity standards.

Person Specification

Qualifications and Experience

* Experience in a health, social care, or community-based role (essential).
* Awareness of the NHS Long Term Plan and ARRS framework (desirable).
* Relevant qualification in health and social care, public health, or counselling (desirable).

Skills and Attributes

* Excellent communication and interpersonal skills.
* Ability to work collaboratively within a multidisciplinary team.
* Strong organisational and time management skills.
* Empathy, compassion, and a patient-centred approach.
* Ability to manage sensitive and confidential information in line with data protection legislation.

Knowledge

* Understanding of social prescribing and its role in supporting primary care.
* Awareness of local and national policies, including safeguarding and equality legislation.

Familiarity with community and voluntary sector services.

Qualifications

* Minimum Level 3 qualification relevant to the Social Prescribing Link Worker role (essential).
* Training or certification in health, social care, or counselling (desirable).


Experience

* Experience working in a health, social care, or community setting (essential).
* Experience supporting people with complex social needs (essential).
* Familiarity with the ARRS framework and NHS personalised care model (desirable).
