Therapy Assistant Practitioner – In-Patient Rehabilitation – Hazelwood Ward, Isebrook Hospital, Wellingborough
The post holder will work in an integrated multi-professional inpatient team providing quality care for a group of patients, specialising in rehabilitation and sub-acute care for patients stepping down from acute hospitals and step up patients from the community in a physical health inpatient unit.
You will be part of an integrated multidisciplinary team including senior nurses, staff nurses, HCAs, physiotherapists, occupational therapists, therapy assistants, medical staff, discharge coordinators and wider external colleagues. The service focuses on delivering vital intermediate care for patients in Northamptonshire.
The role will require covering weekends and bank holidays on a rota basis.
Main duties of the job
We are looking for a proactive and dedicated team member who is open to new ideas and committed to delivering high-quality patient care. Responsibilities include:
* Conduct initial therapy assessments and set therapy goals with patients
* Plan therapeutic interventions and reassess needs with indirect support from Registered Therapists
* Deliver therapeutic interventions including personal care, toilet programmes, dressing assessments and practice, moving and handling, and equipment demonstration to patients and carers
* Initiate therapy group sessions and perform basic home assessments with patients
* Order equipment and perform activities such as kitchen assessments and practice
* Communicate with patients (including those with communication difficulties), families and colleagues; document care on a computerised system
* Involve in induction of junior colleagues and provide support and guidance as necessary

Qualifications
* Foundation Degree in a health subject or equivalent experience
* Functional skills level 2 in literacy and numeracy or equivalent
