Positive Behaviour Support Worker - CJP Outreach Service
We are currently recruiting Positive Behaviour Support Workers to join our team as we prepare to welcome a fourth client to our service. It is advantageous to have a background in Positive Behaviour Support, supporting people with learning disabilities, autism, and behaviours of concern. Experience in community-based settings such as supported living or residential care is preferred.
Role Overview: As a support worker, you will provide person-centred support to adults with complex behavioural needs and communication difficulties, helping them achieve their goals. You will assist in developing essential life skills and promoting independence.
You will be part of a diverse and inclusive team that values support and resilience.
Your responsibilities will include:
* Providing person-centred support tailored to individual needs.
* Collaborating with clients to create and implement support plans.
* Applying Positive Behaviour Support strategies to enhance quality of life and reduce challenging behaviours.
* Assisting clients with daily living skills such as cooking, hygiene, budgeting, and household management.
* Supporting medication management, medical appointments, and health monitoring.
* Facilitating community engagement and social activities to improve clients' social lives and access to services.
* Maintaining accurate care records, documenting progress and incidents.
* Working closely with colleagues, families, professionals, and agencies to ensure holistic care.
Working Hours: Support is provided 24/7 with staffing ratios of 1:1 and 2:1. Shifts are 12 or 15 hours, including alternate weekends and sleep-in duties.
