West Walk Surgery have an exciting opportunity for someone to join our caring and professional Care Navigator team working 18 hours a week.

Days to be discussed at interview but you must be available for one 7.30am start and either a 8pm finish on a Monday or a 6.30pm finish on a Thursday.

We also ask that you are able to offer holiday/sickness cover where needed.


Main duties of the job

We are looking for a happy, caring person to assist our patients in being directed to the correct clinician for advice/treatment working alongside our GP led KLINIK team.

You will be Care navigating by answering phone calls, dealing with patients face to face, sending messages to patients and answering queries sent through our clinical system KLINIK.

We have an established, friendly Care Navigator team waiting to welcome and train you.


About us

West Walk Surgery is a large friendly practice, consisting of 5 GP Partners, 5 salaried GP's, Mental Health Nurse, Pharmacy team, Paramedics, Nurse Practitioner, Practice Nurses and HCA's, all supported by the Care Navigator and Admin teams.


Job responsibilities

Job Summary:

Toensure that the Practice runs efficiently and that a good service is providedto patients, doctors, colleagues and other agencies.

Toparticipate as part of your team. To carry out your duties efficiently andcompetently. To ensure that patients are attended to quickly and in a friendlymanner. To ensure that duties required by Partners or senior staff are carriedout swiftly. To assist in the provision of a pleasant working environment forall staff.

Towork at both the Yate and Coalpit Heath branch surgery.

Duties andResponsibilities:

Work with our GP led KLINIK team.

Makefuture appointments for patients with doctors, nurses and otherclinicians.

Navigatepatients to the most suitable person to deal with their problem.

Providegeneral information such as results of tests carried out.

Registerregular, temporary, immediately necessary or emergency patients using theapproved guidelines.

Takevisit requests according to guidelines, ensuring you have obtained the patientsname, address, telephone number and message.

Generalreception administration

Whenreceiving patient samples, record on computer and take to treatment room.

Whenrequired chaperone a doctor when they are examining a patient.

Processsick note certificate requests.

Amendmentsand deductions

Printamendments and deductions summary and accept the requests.

Private medicalwork

Processincoming and outgoing medical requests.

Liaisewith outside agencies and patients in connection with medical report requests.

Havea good working knowledge of all office equipment and use as required andinstructed.

Respondto the call assist system as per the guidelines.

Usethe panic button as per the guidelines.

Attendpractice meetings as required.

* In the course ofseeking treatment, patients entrust us with, or allow us to gather,sensitive information in relation to their health and other matters. They do so in confidence and have theright to expect that staff will respect their privacy and actappropriately
* In theperformance of the duties outlined in this Job Description, thepost-holder may have access to confidential information relating topatients and their carers, Practice staff and other healthcareworkers. They may also have accessto information relating to the Practice as a business organisation.All such information from any source is to be regarded as strictlyconfidential
* Informationrelating to patients, carers, colleagues, other healthcare workers or thebusiness of the Practice may only be divulged to authorised persons inaccordance with the Practice policies and procedures relating toconfidentiality and the protection of personal and sensitive data

The post-holder will assist in promoting andmaintaining their own and others health, safety and security as defined in thePractice Health & Safety Policy, to include:

* Using personalsecurity systems within the workplace according to Practice guidelines
* Identifying therisks involved in work activities and undertaking such activities in a waythat manages those risks
* Making effectiveuse of training to update knowledge and skills
* Usingappropriate infection control procedures, maintaining work areas in a tidyand safe way and free from hazards

Experience

* Practical Experience of working with others.
* Experience of using own initiative.
* Experience of Customer service.
* Flexibility.
* Confidentiality.
* Experience of working in a GP Surgery.
* Care Navigation trained.


Qualifications

* GCSE passes in Maths and English.
* Care Navigation trained
