We have an exciting opportunity to join our award nominated integrated respiratory team on a fixed-term full-time contract.We strongly encourage applications from physiotherapists, paramedics and occupational therapists with an interest in community/chronic disease management or desire to learn more.
The Integrated Cardio-Respiratory Services is a combined service working across hospital and community pathways, providing community services in clinic and home-based settings. The service is a consultant led multidisciplinary team providing a range of services.
The aim is to provide a more accessible, efficient, cost-effective service integrating respiratory services across secondary and primary care, to enable continuity of care and give respiratory patients the opportunities to have access to care closer to their homes and end of life care in their homes. You will have the opportunity to work within the different specialisms and settings within the community respiratory team.
The successful candidate will be responsible for overseeing service development and progression. They will be proactive in service evaluation and promoting positive change. They will be responsible for the overall pathway management (dependent upon rotation) and wil assist the service lead in ensuring evidence based practice is promoted and followed.
Main duties of the job
1. To facilitate and manage the provision of high quality physiotherapy/nursing care to all adult respiratory patients alongside the consultants, AHP’s and nurses in respiratory medicine in a variety of settings to promote respiratory health within Imperial Healthcare College and the surrounding community.
2. The respiratory practitioner will provide support and advice to doctors, nursing staff, allied health care specialists, patients and their families/carers, known to the community service, and ensure smooth transitions of care into and within the community setting.
3. The respiratory practitioner will have the opportunity to rotate through different specialisms and settings within the Integrated respiratory service.

Education / Qualifications
Essential criteria
4. Degree (or equivalent) in Physiotherapy/Nursing
5. State registration with the Health & Care Professions Council/NMC.
6. Evidence of CPD through maintenance of a portfolio
Desirable criteria
7. Post graduate study in Respiratory Care
Experience
Essential criteria
8. Significant post registration experience in specialty area and experience in either junior specialty role or team leader management role
9. Experience with managing patient relevant to specialist area
10. Experience of working in Pulmonary rehabilitation
11. Experience in integrated respiratory care
Desirable criteria
12. Experience in oxygen services and acute management of the unwell respiratory patient in the community setting.
Skills/Knowledge/ Abilities
Essential criteria
13. Able to demonstrate highly developed specialist knowledge of own patient group
14. Knowledge of research and audit including implementation of findings and review of outcomes
15. Able to manage difficult situations effectively.
16. Demonstrate the ability to compile complex written information
