Working effectively within the team, you will provide evidence-based interventions. You will manage a caseload and undertake assessments and reviews. Using the Care Programme Approach, you will act as a care coordinator, communicating effectively with those involved in the care of people in custody up to release. The role requires you to be a reflective practitioner, eager to work collaboratively with individuals, carers, and other professionals to contribute to the delivery of an effective, dynamic, integrated mental health and substance misuse service. You will work with individuals from initial assessment through to discharge, which can include transferring care to other providers such as hospitals or CMHTs, working with various agencies in the Offender Health Pathway. You will help to rehabilitate and support service users to live hopeful and fulfilling lives as members of the community.
Additional responsibilities include:
1. Taking a recovery-focused and holistic approach in working with service users.
2. Delivering care and interventions utilizing the stepped care model, including the provision of the CPA Framework where appropriate.
3. Assessing people's needs to develop, implement, and evaluate personalized care plans.
4. Facilitating therapeutic group work, taking the lead where appropriate.
5. Delivering evidence-based interventions.
6. Identifying individuals requiring hospital treatment and facilitating timely transfers under The Mental Health Act.
7. Line management of a Band 5 nurse where appropriate.
Some weekend working is required as part of this role.
