Job summary
The Advanced Clinical Practitioner (ACP) will use specialist knowledge and skills to provide healthcare independently to patients.
Thepost holder will demonstrate courageous and critical thinking and is a caring, compassionate and committed experiencednurse who, acting within their professional boundaries, will provide care forthe presenting patient from initial history taking, clinical assessment,diagnosis, treatment and evaluation of care.
They will demonstrate safe,competent clinical decision-making and expert care, including assessment anddiagnostic skills, for patients within the general practice. The post holderwill commit to demonstrating critical thinking in the safe clinicaldecision-making process. They will communicate and work collaboratively withthe general practice team to meet the needs of patients, supporting thedelivery of policy and procedures, and providing nurse leadership as required.
They are able to independentlyprescribe appropriate medication, evaluate or refer to other specialists ifnecessary.
Main duties of the job
1. Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis
2. Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
3. Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs
4. Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)
5. Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
6. Prescribe and review medication appropriate to patient needs and in accordance with national and practice protocols, and within scope of practice
7. Work with patients in order to support compliance to prescribed treatments or over-the-counter medication
8. Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations
9. Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation

The following are the core responsibilities of the Advanced Clinical Practitioner (ACP). There may be, on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:
10. Develop, implement and embed health promotion and wellbeing programmes
11. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
12. Implement and evaluate individual specialised treatment plans for chronic disease patients
13. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects to the patients health
14. Provide advanced, specialist nursing care to patients as required in accordance with clinical based evidence, NICE and the NSF
15. Undertake the collection of pathological specimens
16. Request pathology services as necessary
17. Process and interpret pathology and other test results as required
18. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
19. Maintain accurate clinical records in conjunction with extant legislation
20. Ensure read codes are used effectively
21. Maintain chronic disease registers
22. Develop, implement and embed well woman clinics
23. Develop, implement and embed well man clinics
24. Develop, implement and embed an effective call/recall system
25. Be responsible for the implementation of an effective immunisation programme
26. Chaperone patients where necessary
27. Prioritise health issues and intervene appropriately
28. Support the team in dealing with clinical emergencies
29. Recognise, assess and refer patients presenting with mental health needs
30. Implement vaccination programmes for adults and children
31. Be an extended and supplementary prescriber, adhering to extant guidance
32. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required
33. Contribute to practice targets (QOF etc.), complying with local and regional guidance
34. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
35. Delegate clinical responsibilities appropriately (ensuring safe practice and the task is within the scope of practice of the individual
36. Support the clinical team with all safeguarding matters in accordance with local and national policies
37. Deliver opportunistic health promotion where appropriate

Qualifications
Essential
38. Holds a Post Graduate Degree Qualification in Clinical Practice degree (level 7) or equivalent qualification, with evidence of academic study.
39. Registered Healthcare Practitioner
40. Independent prescribing qualification

Experience
Essential
45. Experience of practice within the four pillars
