Responsibilities
The Health Visitor (SCPHN) will lead a defined caseload and deliver the Healthy Child Programme and national high impact areas within a progressive universal model. The role requires advanced assessment, clinical decision-making and leadership skills to identify need, manage risk and deliver targeted interventions to children and families. The post holder will co‑ordinate care across a skill‑mixed team, providing supervision, delegation and support to ensure safe and effective service delivery. Working in partnership with multi‑agency colleagues, the Health Visitor will contribute to safeguarding, early intervention, health promotion and service development.
The role includes managing complex workloads, maintaining accurate records, evaluating outcomes, and using data and evidence to inform practice and improve services. The post holder will lead and coordinate the delivery of the Health Visiting Delivery Model and Healthy Child Programme for an identified caseload of children and families within the 0–5 Public Health Nursing Service, providing a specialist, evidence‑based public health service that promotes health, prevents ill health and reduces inequalities through a “universal in reach, personalised in response” approach. The role involves assessing, planning, implementing and evaluating programmes of care with a strong focus on early identification of need, safeguarding and improving outcomes for children, particularly within the first 1001 days. The post holder will work collaboratively with families, communities and multi‑agency partners to ensure effective, coordinated and high‑quality service delivery.
