Job responsibilities

Key Roles and Responsibilities
* Reviewand ensure thorough and accurate oversight in terms of governance of patientPMARS and patient pod boxes on the IPU.
* Recordcontemporaneous records on our electronic medical records system and paperrecords. In addition, to access and record on our other electronic systems,e.g. Vantage, our incident reporting system. This is not an exhaustive list ofour inhouse systems, but keeping up to date through the HUB will assist inassessing and identifying these.
* Providea robust governance approach to support safe prescribing and administrations ofmedications on the IPU.
* Workas part of the Rowcroft Hospice MDT.
* Contributeto the further development of the Electronic Prescribing and MedicinesAdministration (EPMA) system within the IPU as part of our team.
* AttendRowcroft Pharmacy Committee meetings every two months, which typically last 1to 1.5 hours and require preparation time. Recurring topics include reviewingprescribing data from the IPU and Community services, as well as Non-MedicalPrescriber activity. To attend at least three a year.
* Supportstaff development as requested on the IPU and within contracted hours.
* Participatein yearly audits with the Rowcroft CDAO.
* Adhereto Rowcroft policies and procedures, maintain mandatory training and have anannual appraisal with your line manager.
* Takereasonable care for your own safety and the safety of others and adhere to allHealth & Safety requirements, processes and procedures.
* Promotea culture of continuous learning, development, and wellbeing by identifyingyour own learning and development needs, ensuring your mandatory and essentialtraining is up to date, and helping to create an environment that continuallyquestions practice and promotes learning.
* Identify your own learning anddevelopment needs, undertake continuous professional development, activelyengage in clinical supervision to maintain objective and innovative practice,and play an active role in team meetings.

RegistrationYouare required to hold a valid professional registration with the General Pharmaceutical Council and you will berequired to provide proof of this and its renewal. You are also required toinform Rowcroft hospice of any restrictions to your practice or investigations.
Infection Prevention
All Rowcroft Hospice employees in both clinicaland non-clinical roles are required to adhere to theInfection Preventionand Control Policies and make every effort to maintain high standards ofinfection control at all times to reduce the burden of Healthcare AssociatedInfections including MRSA.
You agree to the following responsibilities:
* To decontaminate your hands prior to and afterdirect patient care or contact with the patient's surroundings.
* To take part in mandatory infection controltraining provided.
* To responsibly manage your own infections (otherthan common colds and illness) that may be transmittable to patients, includingto contact Occupational Health for guidance.
InsuranceThe work you undertake on behalfof Rowcroft Hospice requires you to be registered with a professional body andhave the appropriate indemnity insurance in place. In the event that yourregistration or indemnity cover becomes invalid or expires your work forRowcroft will be suspended until your registration or insurance is inplace.
Place of WorkThisrole has been identified as Flexible worker (role could becarried out at a Rowcroft site or at home, subject to the varying needs of therole, will spend planned time at Rowcroft sites). The majority will be onsite.
This will be fully discussed and agreed with thepostholder to meet individual and business needs.
Person Specification
Qualifications
* Qualified Pharmacist with active registration.
* Palliative or end of life qualification
Knowledge and Skills
* Palliative Care prescribing in In-Patient and Community settings
Experience
* Hospice and community pharmacist experience
Personal Requirements
* This role requires an Enhanced DBS Disclosure.
* Leadership through creative problem solving, a positive can-do attitude and a willingness and desire to ensure all who come into contact with Rowcroft have the best possible experience.
* Proven ability to be part of a committed and hardworking team in line with the ethics and values of Rowcroft Hospice, acting in the best interests of Rowcroft at all times.
* Encouraged to volunteer for at least one Rowcroft event each year, if interested and able.
