Position

An opportunity has arisen for 4 permanent Faility Home First Nurses within our Emergency Care Services based at University Hospital Ayr and University Hospital Crosshouse.

What you will do:

* Without supervision but under the direction of the Discharge Team Leader and in collaboration with the wider multi-disciplinary team, the post holder will be accountable for the assessment, development, implementation and evaluation of programmes of work for a defined caseload of frailty patients who meet the Home First Criteria.
* Frailty Home First Nurses will ensure joined-up communication supported by digital tools for seamless handovers across health, care and voluntary services.
* The post holders will adopt a “Home First “ approach, and liaise with health and social care partnership colleagues to provide integrated urgent community response, enablement and intermediate care
* Working in collaboration with Health and Social Work colleagues to ensure minimal goals for the in-patient stay and Predicted Discharge Dates, PDD are set for every frailty patient who is transferred to an in-patient bed. The post holder will follow the frailty patient through their admission journey from emergency care to frailty ward, to provide continuity of care and consistent communication between patient, families, allied health professionals and community services.
* To ensure timely and efficient “transfer of care” of patients happens between Acute Hospital and a wide range of community services, including downstream wards, rehab services and Social Work Department.
* The post holder will work within the Professional Standards held in the NMC Code of Professional Conduct and / or HCPC Register, the post holder will work autonomously in the effective and efficient management of all patients who meet the criteria for Home First.
* The post holder will triage patients via telephone calls, whiteboard meetings, emails and Discharge Hub Huddle by carrying out a comprehensive assessment of the patients on the wards and ensuring every patient is on the appropriate pathway and have ‘minimal goals’ based on ‘Home First ethos’.
* The post holder will be responsible for establishing and assisting the assessment of services required by setting minimal goal targets for all service providers including external agencies and the support required in the community for a safe discharge
* Assist and advise, where required, with community beds and Care Home flow.
* Liaise with MDT and support patients, families and significant others

Knowledge, training and/or experience required to do the job:

You are required to have significant nursing experience within acute nursing environment. Have excellent communication skills and the ability to work both independently and collaboratively with multidisciplinary teams. Experience working with frailty and care of the elderly patients is desirable but not essential.

From 1 April 2026, the working week for NHS Agenda for Change staff in Scotland will be reduced. Full-time hours will reduce from 37 to 36 hours (pro rata for part-time staff) without loss of earnings.
