Role overview
The Specialist Health & Wellbeing Coach provides individually tailored diet, nutrition, and behaviour change advice to service users / participants (adults and/or children, young people, and families) to manage their weight and make sustainable changes to improve their overall health and wellbeing.
The role will involve working 2-3 days in the office and 2-3 evenings a week up to 19:30/20:00 when delivering on site. Hours can be flexed as long as evenings are covered. The role is Monday-Friday and covers sites in Cranford, Hounslow, Feltham, Isleworth. Our office is based in Chiswick Town Hall, W4 4JN.
Responsibilities
* Provide evidence-based dietary and behaviour change services via one-to-one and group-based delivery.
* Ensure operational safety of weight management group programmes.
* Support caseloads to implement experiments, track behaviours, utilise tech-enabled support, and explore wider community provision.
* Deliver medium to high-need nutritional and behaviour change services to empower participants to manage weight and lifestyle behaviours.
* Deliver virtual or face-to-face health coaching group sessions in the community as needed.
* Recruit individuals to interventions and support triage and assessment processes.
* Engage clients with agendas, goal setting, problem solving, motivational interviewing, and signposting to other services when required.
* Deliver group weight management underpinned by behaviour change methodology; ensure information provided is engaging, age-appropriate, informative, accurate, evidence-based, culturally relevant, holistic and actionable.
* Provide outreach support including screening services, community events, external meetings, and roadshows/popup activities.
* Work with quality standards, SOPs, data protection, information security, health & safety and safeguarding requirements; escalate where appropriate.
* Support reporting and data collection aligned with contractual and case management system requirements; ensure efficient caseload management and timely data entry.
Qualifications & Experience
* Essential: AFN registered degree in Nutrition or HCPC registered degree in dietetics with CPD evidence.
* Essential: Experience delivering group-based behaviour change interventions to children, families or adults; experience supporting vulnerable individuals through change.
* Essential: Experience with caseload management using a Case Management System; lesson planning and nutrition/weight management content creation.
* Essential: Data entry accuracy and ability to meet targets and deadlines; continuous professional development in health coaching.
* Essential: Experience in health, community or social care environments; understanding of healthy lifestyle issues.
* Desirable: Health coaching qualification or accredited health coaching programme; direct health coaching delivery; experience with lifestyle interventions (smoking cessation, physical activity, NHS Health Checks, weight management, alcohol reduction).
* Desirable: Experience with long-term health conditions and community settings; ability to manage own caseload; supporting people with additional needs.
Individual Competencies
* Strong understanding of social determinants of health and population-based approaches (segmentation, risk stratification).
* Knowledge of behaviour change principles (COM-B model and Behaviour Change Wheel).
* Excellent oral and written communication; capable of effective work with individuals and groups.
* Understanding of confidentiality and safeguarding principles.
* Ability to engage internal and external stakeholders and manage relationships.
* Strong data management, time management, and proficiency with evolving digital tools.
* Commitment to flexible working, including evenings and weekends, to ensure service availability.
Key Business Priorities
Internal and External stakeholders include colleagues, health division, Maximus central division, local authorities, Integrated Care Partnerships, community and voluntary sector, population served, sub-contractors and partners, and community stakeholders. Also includes venue providers and primary care settings.
