What you will do:
– Conduct comprehensive needs assessments for individuals requiring hospital-based social work support.
– Develop, implement, and review care plans ensuring alignment with the Care Programme Approach.
– Deliver social work interventions that are tailored to the needs of service users and their carers.
– Coordinate with the multidisciplinary team to ensure a holistic approach to care and support.
– Monitor, review and evaluate the effectiveness of interventions to ensure continual improvement in service delivery.
– Provide expert advice and guidance on the Care Management process to your team and agency partners.
