We are looking to expand and develop services for heart failure care across the Vale of York. The role will place you at the front of an innovative service, progressing and developing your knowledge and skills while working alongside committed colleagues.
Responsibilities
* Provide high‑quality expert care to patients with heart failure in York Hospital and the Vale of York community setting.
* Deliver an out‑of‑hospital rapid access heart failure clinic based at the local health centre in central York.
* Run community clinics in various settings across the Vale of York and conduct home visits to housebound patients.
* Participate in the development of a heart‑failure in‑reach model focusing on admission avoidance and early step‑down to community‑delivered care.
* Support the team in meeting the heart failure specialist nurse competency framework set by the British Society of Heart Failure.
* Transport yourself and equipment across the locality during the normal working day, requiring a vehicle, a full and valid driving licence, and ability to travel to numerous addresses.
Qualifications
* Full and valid driving licence and access to a suitable vehicle for business use.
* Willingness to travel to a wide range of local addresses not easily accessed by public transport.
* Current or potential approval from your line manager if applying on a secondment basis.
* Experience and knowledge in heart‑failure nursing preferred.
