Community Nurse Case Manager Role
Work as a Community Nurse Case Manager for a complex care provider, supporting clients to live independently at home and within their wider communities. This community role will be covering the West Yorkshire area.
Role Responsibilities
1. Initial Assessments in the community
2. Training & assessing workers' competencies
3. Working with the wider MDT
4. Attending meetings and making referrals
5. Holding team meetings
6. Managing a team of non-clinical case managers
Candidate Requirements
We are looking for an RGN Qualified Nurse with excellent clinical skills in complex care, including peg feeds, ventilators, and tracheostomy management. Tracheostomy and ventilator experience is essential for this role.
Additional Information
All sites are rated as Good with CQC, with some having elements of outstanding. To apply, you must be a car driver as the role is community-based.
This is a full-time role, mainly Monday to Friday, 9am – 5pm, with some flexibility required. It is home-based with travel across Huddersfield, Wakefield, Kirklees.
The salary is negotiable depending on experience, up to £38,000, plus the opportunity to earn up to £6,000 per year in tax-free bonuses based on targets.
