Advanced Nurse Practitioner, Critical Care Outreach
Band 8a
Permanent 37.5 hours per week
We are looking for an enthusiastic and highly motivated nurse with critical care outreach experience to join our Critical Care Outreach team (CCOT) The successful candidate will providing leadership and clinical supervision as we expand our service to provide 24/7 cross site cover working collaboratively with the hospital at night team.
The successful applicant will join a supportive and dynamic team, leading on delivering high standards of care to acutely unwell patients across our site. Our Hospital at Night and CCOT teams are a nurse led service supported by the consultant team. You will have the opportunity to work both as a team and independently, identifying patients at risk of clinical deterioration or that are deteriorating have their needs prioritised with appropriate assessment, intervention and on-ongoing management. Teaching and delivering education to ward teams is an essential part of the role together with leading on deteriorating patient quality improvement work across site.

Main duties of the job
As an ANP for the critical care outreach service the post holder will deliver:
1. autonomous advanced clinical practice,
2. clinical leadership, supervision and support,
3. structured teaching and mentorship
4. facilitate ongoing education, training and development.
Teaching and delivering education to ward teams is an essential part of the role together with leading on deteriorating patient quality improvement work across site.
5. The service aim is to deliver a critical care outreach service to adult inpatient areas at Epsom and St Helier Hospitals:
6. To ensure appropriate escalation and timely management of critically ill patients.
7. To provide a service which improves recognition and response to deteriorating patients on the wards based on NICE CG50
8. To support the rehabilitation strategy for critically ill patients based on CG83 and the process of step down to ward care of patients with complex care needs.
9. To be a role model and a specialist information resource for nursing and medical staff regarding the care and treatment of critically ill patients
10. To collect audit data associated with the keys responsibilities of the post to develop and improve practise
