About the Role
An exciting opportunity has arisen for a kind and experienced Senior Community Practitioner to join our friendly and dedicated colleagues at Princess Road Clinic in Malton. The Whitby and Ryedale Integrated Community Team provides mental health services to adults across the Whitby and Ryedale areas, with bases in Malton and Whitby.
As a member of the team, you will work alongside registered nurses, social workers, psychologists, consultant psychiatrists, pharmacists, admin staff, and support workers, with access to specialist services across the Trust and strong partnerships with local organisations.

Main Duties of the Job
* Act as a named worker/care coordinator, holding a caseload of patients
* Deliver care to meet identified needs autonomously and collaboratively with the MDT
* Participate in the duty worker rota, supporting patients from referral to discharge
* Provide supervision and mentorship to junior staff and students
* Ensure care delivery aligns with relevant legislation including the Mental Health Act, Mental Capacity Act, Equality legislation, and safeguarding duties
* Contribute to clinical risk assessment and management processes
* Work effectively within a multidisciplinary team
Person Specification
Qualifications (Essential)
* Current professional registration with an approved body
* Evidence of Continuing Professional Development
* For registered nurses: MIP, FLIP, ENB 998 Teaching & Assessing in Clinical Practice or City & Guilds 730 Teaching in Adult Education (must be achieved within agreed timescale)
* 18 months post‑qualifying experience including sign‑off of preceptorship or competency framework
* Recognised sign‑off mentor, clinical educator or equivalent (must be achieved within agreed timescale)
* Key skills in literacy, numeracy, and ITQ Level 2 (or equivalent)
Qualifications (Desirable)
* Post‑graduate qualification in a relevant area
* Leadership or management qualification
Experience (Essential)
* Significant experience working with people with mental ill health in a care environment
* Experience working collaboratively with service users and families/carers
* Providing clinical supervision to individuals or groups
* Mentoring or facilitating students on practice placement
* Experience working in a multidisciplinary team
Experience (Desirable)
* Significant experience working with mental ill health in a community environment
Knowledge (Essential)
* Clinical risk assessment and management and its application in practice
* Understanding of psychological models of care and treatment
* Understanding of relevant legislation (e.g., Mental Health Act, Mental Capacity Act)
Knowledge (Desirable)
* Familiarity with the Trust’s Quality Improvement System (QIS)
Skills (Essential)
* Communicate complex and sensitive information effectively to patients, carers/families, and MDT members
* Work effectively as part of a multidisciplinary team and undertake care coordinator responsibilities
* Provide effective clinical supervision, teaching, training, and assessing in clinical practice
Skills (Desirable)
* Competency in venepuncture
