Working hours: 37 hours per week, full time - fixed term for 12 months
Rate of pay: £30,000

Job Description
This role involves working with pupils who struggle within mainstream education due to their Social, Emotional, Mental Health (SEMH) challenges, behaviour, and poor attendance. Resilience and dedication are key qualities. We seek a highly motivated individual with extensive experience working with young people aged 11 to 16 to join our team as we develop our social prescribing practice in Durham. The Link Worker team provides young people with opportunities and relationships to enable them to flourish in education and life. Through participation in targeted activities and therapeutic support, we re-engage even the most disengaged children and young people (CYP).
Key Responsibilities
1. Deliver outcomes for young people by working with them one-to-one and in small groups.
2. Prepare a menu of provision from the social sector in Durham based on social prescribing and educational enrichment principles.
3. Build trusting relationships with young people, create personalized plans, and connect them to health services, education support, community groups, and VCSE organizations offering cultural and sporting activities.
