Main duties of the job
The successful candidate will deliver high-quality, patient-centered care through advanced clinical practice, including comprehensive assessment, diagnosis, treatment, and follow-up care. Working both independently and collaboratively, the ACP will play a key role in enhancing service delivery and improving patient outcomes.

Job responsibilities
 * Conduct comprehensive patient assessments, consultations, and physical examinations independently.
 * Diagnose and manage a wide range of acute and chronic health conditions.
 * Prescribe medications, order investigations, and initiate appropriate treatments in line with clinical guidelines.
 * Maintain accurate, timely, and confidential clinical documentation.
 * Collaborate closely with the multidisciplinary team to ensure coordinated, holistic care.
 * Contribute to service development, clinical governance, and evidence-based practice initiatives.
 * Support the training and mentorship of junior staff, students, and peers.
 * Participate in audits, quality improvement projects, and ongoing professional development.
 * Ensure compliance with all regulatory, legal, and professional standards.
Person Specification
Experience
 * Proven experience working autonomously in a clinical setting.
 * Advanced clinical assessment, diagnostic, and decision-making skills.
 * Independent prescribing qualification (V300 or equivalent).
 * Strong knowledge of pathophysiology, pharmacology, and evidence-based interventions.
 * Excellent communication and interpersonal skills.
 * Strong organisational, time management, and problem-solving abilities.
 * Ability to work effectively in high-pressure, fast-paced environments.
 * Commitment to continuous learning and best practice.
 * Experience in General Practice and Systmone
Qualifications
 * Registered healthcare professional with valid professional registration.
 * Masters degree in Advanced Clinical Practice (or equivalent).
