Are you a Paramedic Practitioner and non-medical prescriber (V300), or willing to undertake the prescribing module? Do you have community, primary care, or urgent care experience and have worked at an advanced level autonomously? Are you looking to develop your skills in primary care? If so, this is the role for you!

We are seeking Paramedic Practitioners to join our newly developed neighbourhood team.


Main duties of the job

You will play a central role in our primary care network program, supporting vulnerable patients. Your responsibilities will include working proactively with practices, voluntary agencies, and advanced nurse practitioners to identify and care for vulnerable patients in care homes and the community, including those with complex medical issues, palliative care needs, learning disabilities, autism, serious mental health illnesses, or difficulties accessing medical services. You will visit patients, assess their health and wellbeing, develop personalized care plans, and collaborate with patients, families, carers, and multidisciplinary teams in community and secondary care to optimize their quality of life within their medical limitations.

Job Title

Accountable to

Clinical Operations Lead, Business Manager, and Clinical Directors, depending on experience.

Full-time (1 WTE, 37.5 hours/week) or part-time/job share options available.


Job Summary

Supporting vulnerable patients in care homes and their own homes.

You will be integral to our primary care network, working with practices, voluntary agencies, and care coordinators to support vulnerable patients, including those with complex medical needs, palliative care requirements, learning disabilities, autism, or mental health issues. You will visit patients, assess their needs, and develop personalized care plans, working with families, carers, and multidisciplinary teams to ensure the best possible quality of life within their medical boundaries.


Primary Responsibilities

The core responsibilities of the clinical operational lead include, but are not limited to:

* Supporting and caring for community patients
* Assessing, updating, and creating personalized care plans for patients in the local primary care network areas
* Triage and manage acute and non-complex medical needs of housebound, palliative, and vulnerable patients within your scope of practice, referring to GPs or community services as needed
* Collaborating with other service providers to ensure good communication across multidisciplinary teams
* Managing chronic medical conditions and supporting patients with these issues
* Implementing and evaluating treatment plans for chronic disease management
* Identifying and supporting at-risk patients to prevent or manage long-term conditions
* Providing advanced nursing care in accordance with evidence-based guidelines, NICE, and NSF
* Undertaking diagnostic procedures such as ECGs, blood pressure monitoring, observations, and specimen collection
* Requesting and interpreting pathology and test results
* Maintaining accurate clinical records following NHS guidelines


Person Specification


Qualifications

* Registered Paramedic Practitioner
* Postgraduate diploma or degree in Advanced Practice or willingness to undertake training
* Experience in primary care, urgent care, or A&E
* Experience in medical assessment and treatment planning
* Prescribing experience and medication review skills
* Qualified prescriber; if not, development to this role is expected
* Experience as a triage assessment practitioner or similar
* Minor Illness Qualification or willingness to undertake training
* Mentoring and supervision training
