Overview
An exciting opportunity has arisen to join the hospital-based Specialist Palliative Care Team at Northampton General Hospital. We are looking for an experienced and highly motivated nurse who is passionate about palliative and end of life care. You will work as an autonomous practitioner managing a caseload of patients with complex palliative symptoms or support needs, ensuring they have timely access to expert symptom control, information and advice. The role covers a seven-day specialist palliative care service with a minimum of one weekend in four.
Responsibilities
* Deliver holistic palliative care to patients and provide emotional support and signposting for carers, family and friends.
* Lead, develop and supervise practice of those working with palliative care patients within the acute setting, and support Band 6 palliative care nurses.
* Develop, implement and evaluate plans of care to meet the complex needs of patients with malignant and non-malignant life-limiting conditions.
* Utilise advanced communication skills for complex and sensitive conversations; advocate for patients and collaborate with other healthcare professionals to prioritise patient preferences and end-of-life wishes.
* Attend the weekly Specialist Palliative Care multidisciplinary team meeting and contribute to patient assessment and care planning decisions.
* Undertake comprehensive holistic assessments and ensure results inform decision making.
* As a non-medical prescriber, prescribe and review medications appropriate to patient need within evidence-based practice and local protocols.
* Demonstrate advanced nursing practice in planning, implementing and evaluating care; document consultations in hospital notes.
* Monitor and evaluate the effectiveness of care plans and revise as necessary.
* Provide Level 2 psychological support for patients and carers; refer to Macmillan Clinical Psychologist where appropriate.
* Act as a specialist resource for other healthcare professionals on Specialist Palliative Care; offer expert nursing advice and support.
* Demonstrate high standards of holistic care and provide clinical leadership related to Specialist Palliative Care.
* Identify educational needs of patients and carers; support strategies to enhance palliative and End of Life care across the trust.
* Facilitate conversations about Advance Care Planning, End of Life care and Preferred Place of Care/Place of Death with patients and families.
* Collaborate with partner services (e.g., Marie Curie Rapid Discharge Service) to facilitate complex/rapid discharge.
* Maintain concise, accurate patient records and contribute to data collection for service evaluation.
* Undertake/participate in audit or research projects related to palliative and end-of-life care.
Requirements / Qualifications
* Registered Nurse with experience in palliative and end-of-life care.
* Non-medical prescriber status or eligible to undertake non-medical prescribing.
* Strong communication skills and ability to lead and develop others within a multidisciplinary team.
* Experience in advanced decision-making, complex symptom management, and patient/family advocacy.
