Job overview

The post holder is expected to assess, plan, implement and evaluate care of the child/young person (CYP) in the community setting, children's centre and acute paediatric ward. The CYP will have a nursing need, categorised into four main groups.

1. CYP with acute and short term conditions.
2. CYP with long term conditions.
3. CYP with disabilities and complex conditions.
4. CYP with life limiting and life threatening conditions.

The post holder will facilitate and provide safe and effective care working in partnership with the CYP, family/caregivers and the multidisciplinary team, to ensure holistic and family centred care in the most appropriate setting supported by a senior community children's nurse.

The post holder will work autonomously as well as part of a team/integrated agency approach in provision of high quality, evidence based care.


Main duties of the job

To independently manage a designated caseload, working collaboratively with the wider multidisciplinary team to support clinical decision‑making. Provide nursing care within the child or young person’s home or educational setting, including (but not limited to) gastrostomy management, non‑invasive ventilation support, wound care, injections, intravenous therapies, oncology blood sampling, line care, and any other clinically appropriate interventions.

Attend MDT meetings, discharge planning, TAF and CP meetings as required, with support from the line manager. Deliver condition‑specific training to families and education staff in the appropriate setting.

Contribute to the provision of palliative and end‑of‑life care within the home. When required, work alongside a senior team member as part of the 24‑hour service.

* Able to interact well with children/young people, parents, carers, and other professionals
