Job Title: Remote Clinical Pharmacist (Independent Prescriber)
Location: Remote (GP Surgery based)
Hours: Full-time, 37.5 hours per week (Monday Friday)

We are a forward-thinking GP surgery committed to delivering high-quality, patient-centred care. Our supportive multidisciplinary team is looking to expand with the addition of an experienced Clinical Pharmacist to work remotely.
The Role:
We are seeking a motivated and skilled Clinical Pharmacist to join our practice team. The successful candidate will be CPPE-qualified and an Independent Prescriber, able to work autonomously as well as collaboratively with the wider primary care team.
Key Responsibilities:
* Conducting medication reviews and structured medication optimisation.
* Supporting patients with long-term condition management.
* Prescribing safely and appropriately within scope of practice.
* Supporting clinicians with repeat prescribing processes and medicines management.
* Providing expert advice on medicines and promoting evidence-based prescribing.
Requirements:
* CPPE-qualified.
* Independent Prescriber status.
* Previous experience in a GP practice or primary care setting (preferred).
* Excellent clinical decision-making and communication skills.
* Ability to work effectively as part of a multidisciplinary team.
