The Tower Hamlets Memory Clinic provides a dynamic, innovative and responsive service for the population where dementia diagnosis rates are increasing. The clinic provides high quality comprehensive assessments and diagnosis for people experiencing cognitive decline by working actively in partnership with service users, carers and relatives. This includes quality post-diagnostic support and intervention that promotes independence, social inclusion and maintains quality of life. The service works in partnership with other teams, statutory and voluntary agencies as well as universities and other research organisations. You will be responsible for supporting service users and carers through the diagnostic pathway by providing comprehensive cognitive assessments and post-diagnostic support. Responsibilities

Qualifications and Experience
Education/ Qualification/ Training
* Essential: Registered Mental Health Nurse; educated to diploma or degree level; evidence of continual professional development
* Desirable: Higher Education qualification in Mental Health; trained Adult Safeguarding Enquiry officer or willing to undertake Enquiry Officer training; ENB 998/ Mentorship Course or equivalent; evidence of continual professional development related to cognitive impairment/dementia/frailty care
Experience
* Essential: experience of working as a qualified nurse within a Memory Clinic or similar Dementia Diagnostic service; experience of community caseload management; experience working with a diverse community in a complex environment; experience of working as part of a multi-professional or multi-agency team; experience conducting holistic psycho-social assessments and providing comprehensive care and support plans based on individual need
* Desirable: experience in an ethnically diverse inner city area; experience with older adult community; experience in integrated Health and Social Care Team; experience of conducting standardised cognitive tests; experience of facilitating support groups
Knowledge
* Essential: understanding of the Mental Health Act 1983 (as amended 2007); Mental Capacity Act 2005; Care Act 2014; CPA and Recovery Model principles; up-to-date knowledge of current approaches to mental health nursing; risk assessment and management; safeguarding adults and children; clinical governance and evidence-based practice; post-dementia diagnosis support; equal opportunities and Human Rights Act 1998
Skills
* Essential: effective verbal and written communication; prioritising workload; excellent time management; autonomous and independent working; innovative practice; team work; good IT skills; holistic engagement with people with serious mental health problems; ability to administer medications (including intramuscular injections) and de-escalate using PMVA techniques
* Desirable: proficiency in Excel, PowerPoint and Team; ability to use these tools to deliver training and facilitate groups
