Job Description: Care Coordinator

Position Summary

The Care Coordinator is responsible for developing, managing, and reviewing individualized care plans while ensuring the delivery of high-quality, person-centered care. This role involves conducting assessments, coordinating services, monitoring care delivery, and supporting staff to meet client needs. The Care Coordinator also provides oversight on the floor, completes regular audits, and ensures compliance with policies, procedures, and regulatory standards.

Key Responsibilities

* Develop, implement, and regularly update individualized care plans in collaboration with patients/clients, families, and care teams.
* Conduct scheduled reviews of care plans to ensure accuracy, effectiveness, and alignment with changing needs.
* Provide oversight on the floor, monitoring daily care activities to ensure quality, safety, and adherence to care plans.
* Perform monthly audits of care documentation, records, and practices to ensure compliance with organizational standards and regulatory requirements.
* Coordinate referrals to medical, behavioral health, and community services as appropriate.
* Track patient/client progress, monitor treatment adherence, and make adjustments to care plans as necessary.
* Educate patients/clients and families on care plans, health management, and available resources.
* Collaborate with multidisciplinary teams to enhance service delivery and client outcomes.
* Support, guide, and train staff on care planning and compliance requirements.
* Maintain accurate and timely documentation, case notes, and reports.
* Advocate for patients/clients to ensure they receive the highest standard of care.

Qualifications

* Previous experience in care coordination, case management, or healthcare services
* Level 5 or work towards level 5
* Knowledge of healthcare systems, community resources, and regulatory requirements.
* Strong organizational and time-management skills, with the ability to manage a caseload and conduct audits.
* Excellent communication, interpersonal, and leadership abilities.
* Proficiency with electronic health records (EHR) and care management software.

* Strong care planning and assessment skills.
* Attention to detail with auditing and compliance oversight.
* Ability to balance administrative tasks with direct care oversight.
* Problem-solving and critical thinking abilities.
* Empathy, compassion, and active listening.

Work Environment

* Works within healthcare facilities, residential programs, or community organizations.
* Combines office-based care planning with on-the-floor oversight.
* Requires conducting monthly audits and reviews in addition to daily responsibilities.

Job Types: Full-time, Permanent

Pay: Up to £27,000.00 per year
