The role

As part of the NHS England Additional Roles Reimbursement Scheme (ARRS),we are seeking a skilled and motivated AdvancedClinical Practitioner (ACP) to join our Primary Care Network (PCN) team. TheAdvanced Clinical Practitioner (ACP) will be able to assess and treat variedclinical cases, having advanced skills to assess the patient with adifferentiated or undifferentiated clinical diagnosis. Advanced communication,problem-solving and decision-making skills will be used to initially assess andprovide on-going patient care. Working within local and nationally agreedprotocols and guidelines, the ACP exercises independent judgement to assess,investigate, diagnose, plan, implement and evaluate the clinical care andmanagement of patients.

Main duties of the job

We are seeking an experienced and motivated Advanced Clinical Practitioner (ACP) to join our Primary Care Network (PCN). The ACP will provide high-quality, patient-centred care, working autonomously to assess, diagnose, treat and manage patients with a wide range of acute, chronic and complex health conditions.

The successful candidate will undertake advanced clinical assessments, develop and implement treatment plans, prescribe medication where appropriate, and order and interpret diagnostic investigations. They will work closely with GPs and the wider multidisciplinary team to ensure safe, effective and integrated care for patients.

The role also includes providing follow-up care, supporting patients to manage their health, promoting health education and wellbeing, and contributing to the continuous improvement of services. The ACP will support clinical governance, participate in audits and quality improvement initiatives, maintain accurate patient records, and ensure compliance with professional, organisational and regulatory standards.

In addition, the post holder will act as a clinical resource within the team, sharing expertise, supporting colleagues, and contributing to service development. Job responsibilities

The post holder will provide clinical expertise for developing and enhancing the service and will demonstrate advanced skills and competencies and ensure that evidence-based practice is inherent in all aspects of care and treatment.

Key Responsibilities

Clinical Assessment and Care

Provide a level of advanced and comprehensive clinical assessment based on a specialist body of knowledge and advanced clinical reasoning skills.

To appropriately assess, examine, investigate, diagnose and treat a wide range of complex or urgent care patients, resulting in the safe management and appropriate referral of patients with undifferentiated, undiagnosed and complex presentations.

Ensure patients are informed and consent is gained prior to treatment, investigation and management, as appropriate.

Is able to identify the need for appropriate diagnostic tests and interpret the results.

To identify and intervene proactively where circumstances contribute to an unsafe environment for patients, staff or relatives and escape these appropriately.

To share knowledge and expertise, acting as an expert resource to others across primary and secondary care, as appropriate.

Contribute to the development and improvement of systems and processes that facilitate patients being able to improve their health.

To improve the quality of the patient experience by identifying and meeting the individual clinical needs of patients, including issues pertaining to child protection and vulnerable adults.

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of the PCN.

To maintain up to date patient records in accordance with policy, including the use of the Electronic Patient Records System.

Ability to adapt specialist clinical knowledge and skills to different clinical settings and influence service delivery and patient care.

Responsible for rapidly and continuously evaluating the patients condition and response to therapeutic interventions and treatment modalities and modifies the plan of care for optimal patient centred outcomes.

Contribute to the review and development of clinical patient pathways to ensure a standardised approach to care.

Actively implement effective systems and processes for infection prevention and control management and relevant audit within their clinical area.

Educates patients, relatives and staff as necessary ensuring understanding has been interpreted accurately.

Ensures patients views are considered in the decision-making process.

Participates in meeting and promoting the health education needs of patients and carers.

Promotes the health through co-ordinated activities and programmes with patients, colleagues and carers where appropriate.

Practice autonomously as an ACP using collaborative and independent judgement to assess, investigate, diagnose, plan, implement, manage, and evaluate clinical nursing and medical care in the management of patient.

Develop, implement, and evaluate patient management plans in collaboration with patients, their families, and the multidisciplinary team.

Deliver safe, evidence-based, and patient-centred care, ensuring that the individuals needs are met.

Provide follow-up care and support, monitoring progress and adapting treatment plans where necessary.

Leadership and Collaboration

Provide clinical leadership and mentoring to junior colleagues and other members of the clinical team.

Work closely with GPs, nurses, and other healthcare professionals to ensure integrated care delivery.

Promote best practices in clinical care and contribute to continuous professional development and team learning.

Quality and Compliance

Participate in audits, reviews, and quality improvement initiatives within the practice.

Ensure that all clinical activities comply with relevant policies, procedures, and regulations.

Maintain accurate and timely patient records in accordance with data protection laws and practice protocols.

Alert other team members to issues of quality and risk.

Assess own performance and take accountability for own actions, either directly or under supervision.

Contribute to the effectiveness of the team by reflecting on own and team activities and making suggestions on ways to improve and enhance the team's performance.

Work effectively with line management to meet organisational need.

Effectively manage own time, workload and resources.

Training and Development

Stay up-to-date with the latest clinical guidelines, advancements, and best practices in primary care.

Disseminate learning and information gained to other team members in order to share good practice and inform others about current and future developments (e.g. courses and conferences).

Assess own learning needs and undertake learning as appropriate.

Equity and Diversity

Identify patterns of discrimination and take action to overcome this and promote diversity and equality of opportunity.

Enable others to promote equality and diversity in a non-discriminatory culture.

Support people who need assistance in exercising their rights.

Monitor and evaluate adherence to local chaperoning policies.

Act as a role model in the observance of equality and diversity good practice.

Accept the rights of individuals to choose their care providers, participate in care and refuse care.

Assist patients from marginalised groups to access quality care.

Infection Prevention & Control

In accordance with the Health and Social Care Act 2008, the post holder will actively participate in the prevention and control of infection within the capacity of their role. The Act requires the post holder to attend infection prevention and control training on induction and at regular updates and to take responsibility for the practical application of the training in the course of their work. Infection prevention and control must be included in any personal development plan or appraisal.

WORKING CONDITIONS

The post holder will be required to use VDU frequently, for prolonged periods of time.

Person Specification

Qualifications

  • Qualifications & Training
  • Registered Nurse (Adult)
  • Advanced Clinical Practitioner (ACP) or Advanced Nurse Practitioner (ANP) qualification (Level 7 or equivalent)
  • Relevant clinical postgraduate qualifications or experience

Clinical Skills

  • Competent in advanced clinical assessment and diagnostic skills
  • Ability to manage and treat acute and chronic conditions
  • Proficient in prescribing and clinical decision-making

Experience

  • Extensive clinical experience within a primary care, urgent care, or equivalent setting
  • Demonstrable experience in minor illness and chronic disease management
  • Experience of working autonomously and within a multi-disciplinary team
  • Leadership or mentoring experience

Knowledge & Expertise

  • Sound understanding of national clinical guidelines and healthcare policies
  • Knowledge of safeguarding, mental health, and long-term condition management

Other

  • Right to work in the UK
