What you will do Manage a defined caseload under the Care Programme Approach (CPA), delivering comprehensive mental health assessments, risk assessments, and evidence-based interventions. Develop, implement, and review person-centred care plans in partnership with service users, carers, and partner agencies, promoting recovery and independence. Act as Care Coordinator, ensuring integrated care delivery across health and social care systems, including liaison with GPs, social services, and voluntary sector providers. Participate in MDT meetings, contributing to clinical decision-making, safeguarding processes, and service development initiatives. Person Specification Qualifications Registered Mental Health Nurse (RMN) with degree-level qualification or equivalent. Evidence of continued professional development (CPD). Mentorship or practice assessor qualification (desirable). Registration & Compliance Current registration with the Nursing and Midwifery Council (NMC). Working knowledge of the Mental Health Act 1983 (amended 2007), Mental Capacity Act 2005, and Care Programme Approach.
