Job Summary
Bury St Edmunds PCN has an opportunity for a care coordinator to join our primary care network (PCN), supporting five GP practices across the Bury area. The role is full-time, 37.5 hours per week, mainly Monday and Friday from 08:30 to 18:00, with some weekend hours depending on demand.
The role involves close collaboration with practices, the multidisciplinary team (MDT), and the wider PCN. It is crucial in ensuring patients receive optimal care, working on requirements for the PCN Direct Enhanced Service Contract (DES).
Our care coordinators proactively identify and support vulnerable groups, including the frail/elderly and those with long-term conditions, facilitating care coordination and navigation across health and social care services.

Main Duties
1. Work with patients, families, and carers to improve understanding of health conditions and develop personalized care plans.
2. Assist patients in managing their needs through queries, appointments, and providing clear information about their care options.
3. Collaborate with GPs and primary care professionals to manage a caseload, including patients with long-term health conditions.
4. Support multidisciplinary team coordination within the PCN.
5. Promote shared decision-making and prepare staff and patients for these conversations.
6. Develop communication channels between GPs, patients, families, carers, and other agencies.
7. Maintain records of referrals and interventions for monitoring and evaluation.
8. Update and maintain accurate care records.
9. Contribute to risk assessments and service evaluations.
10. Work with commissioners and local agencies to develop the role further.
