What you will do:
* Conduct comprehensive Care Act assessments and reviews across various service areas including Domiciliary Care, Supported Living, 24-hour placements, and Extra Care Housing.
* Perform CHC checklists and evaluate eligibility for Continuing Health Care, ensuring compliance with relevant legislation such as the Care Act 2014 and Mental Capacity Act 2005.
* Undertake Mental Capacity Assessments (MCAs) and facilitate best interest decisions, providing informed recommendations for individual care pathways.
* Engage in safeguarding procedures with robust knowledge and practical skills, advocating for the safety and well-being of adults in care.
* Collaborate with multidisciplinary teams in Continuing Health Care assessments and attend CHC/MDT meetings as an integral team member.
* Carry out face-to-face visits to clients, offering dedicated support and engaging with individuals to ensure their needs and preferences are met comprehensively.
