The South Mental Health Social Work Team is focused on supporting and empowering individuals to achieve the best possible mental well‑being, reach their goals, and maximise the quality of life of those we support. We work closely with colleagues in health, commissioning, safeguarding and partner agencies to create creative care plans, manage risk in the community and support discharge plans from out‑of‑county and rehab mental health wards. We support individuals from 18+ with functional mental health conditions and co‑morbidities, and support the transition process from children to adult services when required.
We are passionate about the development of staff within the team and are a supportive, dynamic, and person‑centred team of practitioners. At present we have staff undertaking apprenticeships in the team, staff completing their ASYE and those undertaking their AMHP training, as well as staff who have champion roles in particular areas of interest to them. We encourage regular reflection with fortnightly reflective practice sessions and have the Social Work Academy who offer a range of training opportunities. We also have practice educators in the team and regularly welcome students on placement.
