The Community Nursing Service inNorth East Lincolnshire works in liaison with GPs Practice nurses and otheragencies to provide a high quality, skilled and holistic nursing care serviceto the community.
Main duties of the job
Team: Community Nursing - Meridian Team
Hours: 31 hours
Permanent
Salary Range 3 - £24,968 a year pro rata, appointed on fixed point
You will work on your own out in the community,under direction of the team, to provide a range of care needs to our clients, such as wounddressings, eye care, pressure area care, venepuncture, monitoring of blood pressure, weightand blood sugar as well as palliative and terminal care.You will also provide emotional support to patients, carers and family memberstherefore an empathetic and sympathetic approach with effective communicationskills. Most importantly you must beable to demonstrate your commitment to the values of Care Plus Group.
You must hold a relevant qualification, i.e. QCFLevel 3 in Health and Social Care. A Level 2 qualification in Numeracy andLiteracy is also essential or an ability to demonstrate an equivalent level ofcompetence. You will have accurate and proven record keeping and ITskills.
In returnyou will have access to further training and development with ongoingsupervision and support.
You will transport yourself and equipment acrosslocations within North East Lincolnshire to deadlines and you will be required towork evenings, weekends and bank holidays on a rota basis showing a flexibleapproach to working arrangements.

Qualifications
* NVQ Level 3 in Health and Social Care
* Numeracy and Literacy Level 2
Experience
* Experience of implementing and evaluating care with clients
* Experience of working with clients in a Community Setting

* Ability to work on their own in the community setting
