An exciting opportunity has arisen at Atkinson Health Centre Practice for the recruitment of a highly motivated and enthusiastic Practice Nurse to join our welcoming and well-supported team for 23.5 hours per week over 4 days.
We are a long-established, friendly and supportive training practice, located in the Alfred Barrow Health Centre, purpose built and holds 3 GP practices, a number of community services and a Boots pharmacy. We are seeking to appoint a clinically skilled and patient-focused Practice Nurse to join our happy and committed clinical team.
Main duties of the job
Providing vaccinations/immunisations and travel medicine, wound care, requesting necessary pathology services with subsequent result processing. Conducting chronic disease clinics and supporting patients with Diabetes, Asthma, COPD and Cardiovascular diseases, delivering patient care and making referrals to secondary/specialist care when required. Prioritizing health issues, recognising, and referring patients when required to secondary care services, and supporting patients in medication use.
Health Promotion: Support health promotion initiatives within the practice by providing information to patients on healthy lifestyle choices and encouraging patients to engage in preventive healthcare measures. Contribute to patient education programmes within the practice.
Infection Control: Adhere to infection control protocols to maintain a safe and clean environment for patients and staff. Assist in maintaining the cleanliness and organization of clinical areas.
Collaborate with healthcare professionals, including GPs, nurses, and administrative staff, to ensure smooth operations within the practice.
Job responsibilities
Key Responsibilities:
Responsible for the delivery of practice nursing services, working as part of the practice team.
To provide support to GPs in the management of chronic diseases such as Diabetes, COPD, Asthma, Hypertension, Heart Failure etc. by providing specifically defined clinical sessions, working closely with the GPs to manage patients overall care.
To provide wound care, dressings, including pressure bandaging and higher level wound care, liaising directly, where appropriate with community and secondary care counter-parts, escalating to a GP as necessary.
To undertake Cervical Cytology sampling and administer immunisations, including travel medicine and contraceptive medicine as outlined by national programmes and local guidelines, working closely with the nursing, clinical and admin team to create systems to identify patients who are in need of such services and promoting the service hard to reach patients.
Manage appointments, accurate maintenance of clinical records, effective use of SNOMED codes, and the management of chronic disease registers.
The responsibilities include identifying, managing, and supporting patients at risk of developing long-term conditions to prevent adverse effects on their health.
Support health promotion initiatives within the practice by providing information to patients on healthy lifestyle choices and encouraging patients to engage in preventive healthcare measures. Contribute to patient education programs within the practice.
Adhere to infection control protocols to maintain a safe and clean environment for patients and staff. Assist in maintaining the cleanliness and organization of clinical areas.

Experience
* Experience of EMIS clinical system and QoF
* Experience of working in Primary Care
* Experience of working in GP Practice
Qualifications
* Registered Nurse qualification and registration with The Nursing and Midwifery Council with a valid NMC PIN
* Qualification in Chronic Disease Management - Diabetes, Respiratory, Cardiovascular
* Competent in basic nursing duties required for the post
