Are you ready to embark on a fulfilling journey in the world of social work? We are seeking an experienced Social Worker to join our Hospital Discharge Team. Positioned in three acute hospitals, our teams provide crucial advice, assessments, and discharge planning, ensuring safe and timely transfers of care through a collaborative, multi-disciplinary approach. As a key team member, you will conduct complex assessments, serve as an Investigating Officer in safeguarding cases, and apply expertise in the Mental Capacity Act 2005 and the Care Act 2014. Bring at least 2 years of post‑qualifying experience and Social Work England registration, confidently offering peer supervision and support to fellow social workers and Social Care Practitioners. Your commitment, coupled with first‑class knowledge of relevant legislation, will drive the positive change we aim to achieve.


Job Purpose

To undertake social work / social care assessment with customers to support the discharge planning process. To plan for the provision of support and / or services with customers in order to assist them meet their outcomes and to live as independently as possible fulfilling their individual potential. To support carers to continue in their role. To deliver this service in accordance with the statutory responsibilities as set out within NHS & Social Care legislation; and the eligibility criteria for the service as agreed by Wiltshire Council. To ensure that the primary responsibility of safeguarding vulnerable adults is delivered in accordance with the law and local policy and guidance. To provide professional supervision and support to Level 2 Social Workers and Customer Coordinators, working within the same locality / environment. To uphold standards of practice for Social Workers as registered with Social Work England and in accordance with Codes of Conduct.


Responsibilities

* Undertake person‑centred assessments, including Mental Capacity Assessments, with customers and plan for the provision of support and/or services to assist them meet their outcomes and live independently.
* Ensure accurate and timely recording of all information and assessment activity onto the Social Care records database in accordance with work targets set by the Hospital Team Manager and Supervisor.
* Gather sufficient information about customers to accurately assess their needs against eligibility criteria.
* Keep up to date local knowledge of universally available services, including voluntary and community sector, and signpost/advocate for customers accordingly.
* Apply Council policy to assess and manage risks for individuals and support customers to manage risk appropriately.
* Identify where vulnerable adults may be at risk and act in accordance with the Department’s safeguarding policies and procedures.
* Act as Investigating Officer in safeguarding cases following Department policies and procedures.
* Participate in Specialist Response/Duty rota to enable access to services and make decisions about risk, eligibility and priority.
* Work as part of a multidisciplinary team, with health and social care colleagues, to ensure effective communication and support assessment/decision‑making processes.
* Follow Council corporate and departmental Policies and Procedures at all times.
* Contribute proactively to the effective working of the team with a positive attitude, sharing knowledge, offering advice and support, and participating in service development.
* Engage in self‑evaluation and supervision to ensure continuous professional development and evidence of CPD.
* Represent the Council positively, professionally and appropriately at meetings with customers, external partners and agencies.


Qualifications – Essential

* At least 2 years of post‑qualifying experience as a Registered Social Worker (Social Work England). Social Work England registration required.
* Evidenced experience of professional and personal development following achievement of professional qualification.
* Evidence of informal supervision and the ability to provide peer supervision.
* Strong interpersonal skills and effective communication with people at all levels.
* Proficiency with Microsoft Office and Electronic Social Care record software.
* A self‑starter, well‑organised person who is passionate about delivering high‑quality customer‑centric services.
* Ability to collaborate with others in a developmental manner.
* Strong written communication skills, able to produce clear, complex and accurate reports.
* Fluent in English.
* Up‑to‑date knowledge of relevant legislation and guidance on safeguarding vulnerable adults.
* Knowledge and understanding of Health & Social Care legislation and wider policy context.


Qualifications – Desirable

* ECDL (European Computer Driving Licence).
* Evidence of formal supervision and appraisal.
* Qualified Practice Teacher.
* Additional experience or training in relevant areas.
