Job responsibilities MAIN ROLE AND PURPOSE OF THE POST The Clinical Pharmacist will work across Sparcells PCN. They will be part of the clinical team and focuses on all aspects of medicines management. They will support patient care by focussing on prescribing and ensuring that it is done in a safe, efficient and timely manner. SPECIFIC RESPONSIBILITIES Repeat Medication The pharmacist will be an independent prescriber, able to initiate and continue medication. They will: Review requests for repeat medication and where appropriate authorise issues of medication, ensuring that relevant checks and reviews have taken place. Ensure that regular medication reviews are carried out with patients. Support the admin team with queries on repeat medication requests. Support the clinical team with advice on medication. Liaise with pharmacies as necessary. Hospital Correspondence When patients are seen in secondary care they send general practice details of their findings, any changes to medication and care plans. Upon receipt the pharmacist will: Review all hospital discharges, check medication against current repeat items and where appropriate amend patient medication in respect of any changes. Review outpatient letters, check medication against current repeat items and where appropriate amend patient medication in respect of any changes. Liaise with hospitals as necessary to resolve inconsistent, illegible or spurious information. Add coded information to the patient electronic record as necessary. Medicines Management We work with BSW ICB to ensure that our prescribing remains safe, effective and provides value for money. The Clinical Pharmacist will support this work by: Reviewing patients with polypharmacy and make recommendations. Work with the practice Prescribing lead to achieve all aspects of the Prescribing Incentive Scheme. Initiate audits of prescribing and feed back to the team as necessary. Monitor the reports from the Eclipse System and take action as appropriate. Action medicines safety alerts. Participate is the care of patients with chronic diseases through the Quality and Outcomes Framework. GENERAL RESPONSIBILITIES Confidentiality You shall comply with the General Data Protection Regulations when processing personal data for patients and staff. You must be aware of the Caldicott Principles as they affect your work. You can address any questions in relation to this to your line manager. Any data breaches should be reported immediately to the Data Protection Officer. All enquiries from the media must be referred to a Director. Health and Safety All employees have a statutory duty of care for the safety of themselves and others. Employees are required to report any hazards as soon as they are identified. All staff must contribute to the management of risk. This requires them to assess the risk in all situations, take appropriate actions to mitigate any risks identified and report all hazards, incidents and near misses. All buildings occupied by staff are non-smoking environments. No-one may smoke or vape in the building. Staff who smoke or vape outside the buildings should cover their uniform.
