An opportunity has arisen for dynamic leader who can evidence personal and organisational development to apply for the post of Critical Care Outreach Practitioner.

Applicants must have a 1st level degree as essential criteria and willingness to work towards masters degree. They will act as a clinical expert utilising well grounded academic skills to lead the service into the future, demonstrating advanced specialist skills and knowledge, whilst assuming professional responsibility and accountability in the assessment and treatment of patients in their care.

Main duties of the job

The post holder's will support and improve services for patients in ward areas who are at risk of deteriorating and may require admission to level 2 or 3 critical care areas. In addition to this discharge from Critical Care areas will be supported by the Critical Care Outreach team ensuring post ICU patients receive appropriate physical and psychological support in line with national guidance.

The applicant's must demonstrate exemplary interpersonal skills and have the ability to effectively communicate with all members of the multi-professional team. Their role will epitomise the promotion and facilitation of good practice to ensure the delivery of high standard quality patient care. There is an expectation that the Critical Care Outreach Practitioner will lead and advance practice in their speciality area whilst participating in clinical facilitation and education regarding the recognition and response to the at risk or deteriorating patient. They will actively participate in audit in order to strategically inform future care delivery.

Qualifications

Essential criteria

1. Registered professional healthcare qualification
2. First level degree
3. Masters degree or working towards
4. Physical assessment skills level 6/7
5. Non- Medical Prescribing or working towards Non- Medical Prescribing within a pre-determined timescale
6. Advanced Life Support provider
7. Evidence of leadership in the specialist area
8. Commitment to undertake an intensive in-house course including competency assessment by Consultants in Intensive Care Medicine
9. Commitment to undertake Outreach Competency Programme/ bespoke course for the establishment of competence and confidence in the core skills to enable the delivery of Critical Care Outreach.

10. Relevant Critical Care Course at Diploma/Degree level

Knowledge

Essential criteria

11. Ability to apply advanced theoretical knowledge to the clinical setting.
12. Specialist knowledge of a wide range of clinical specialities as the service covers all areas of the hospital. The post holder may be called to any area to assess at risk patients.
13. Ability to provide a rapid response and support system, using expert knowledge and in-depth experience to identify deterioration of patients physical condition
14. Ability to provide highly specialised interventional advice with appropriate physical assessment, observation and treatment for acute patients on the ward facilitated by the NEWS track and trigger system
15. Demonstration of critical thinking, problem solving and decision making skills to analyse and judge the most appropriate actions to take when dealing with patients with complex medical problems.
16. Ability to transfer and adapt skills from previous knowledge and experience
17. Facilitate the development of junior staff providing support and assistance in their own professional and academic development
18. Effective verbal & non-verbal communication and interpersonal skills
19. Ability to co-ordinate work within a multi-professional team
20. To assist other staff members in their time management and ability to prioritise care.
21. Demonstrate a micro and macro problem solving approach
22. Demonstrate advanced leadership skills
23. Service improvement and change management experience
24. Proven managerial skills
25. To act as an autonomous practitioner within professional and clinical guidelines
26. Reporting and processing of minor incidents and complaints
27. Maintain a high standard of clear and concise documentation and record keeping within the clinical environment

Experience

Essential criteria

28. Significant critical care experience or acute care experience in an acute care setting.
29. Significant experience as a senior healthcare professional within a leadership role
