You will be a crucial member of our Adult Community Team, working with adults aged 18 and over to support their journey towards independence and well-being.

Responsibilities
* Undertake Care Act Assessments to assess and support the needs of adults in the community.
* Carry out Mental Capacity assessments and Best Interest outcomes, ensuring decisions are in the best interests of individuals.
* Complete Deprivation of Liberty Safeguards (DoLS) assessments with proficiency.
* Navigate Continuing Healthcare (CHC) checklists and Decision Support Tools (DSTs) as part of assessment processes.
* Work from a Strengths-Based perspective and demonstrate Least Restrictive Practice in all interventions.
* Support the team duty function by triaging referrals and prioritising daily case management activities.
* Collaborate confidently with professional partners in a multidisciplinary team (MDT) approach.
