Community Nursing teams are based in the following hubs:
* North Hub – Haxby Health Centre
* Selby/South Hub – Selby Hospital, Tadcaster Health Centre or Sherburn in Elmet
* East Hub – Tang Hall Clinic
* West Hub – Acomb Health Centre
* South Hambleton and North Ryedale – Easingwold, Pickering and Kirkbymoorside
We currently have fixed‑term and permanent vacancies at SELBY/SOUTH. Team members may be required to work in a hub that differs from their base.
We are a large, supportive, friendly team that works independently while always having the support of the team when needed. We have been recognised many times by the Trust as a fantastic team and a great asset to the organisation.
The service runs 7 days a week from 7:30 am to 8 pm. No overnight shifts are required, but flexibility around availability on weekends is required. Night shifts are currently provided by a dedicated overnight team, but there is an opportunity to rotate to night shifts if interested.
CPD is expected and you will have the opportunity to learn and teach others to meet the needs of patients in their own homes.
Main duties of the job
We are community nursing teams comprising motivated professionals, working out of the hospital setting to deliver nursing care in patients’ homes and community settings. We work closely with our hospital, social and primary care colleagues to support patients to stay in their homes and avoid hospital admission.
Geography varies depending on your base across York, Selby District, Tadcaster, Pickering area and Easingwold. Each day you will work autonomously, delivering patient care to meet patient needs holistically. You may be based at one of our community health centres.

It is essential that you are a car driver and have access to a vehicle for work. Your mileage will be covered; parking at our community sites is free for clinical staff.

Person Specification
Knowledge & Skills
* Post registration experience of working within Community or hospital services
* Experience of clinical supervision
* Relevant skills such as palliative care experience, care of older people
Education & Training
* Current NMC registration
* Holds or prepared to work towards gaining an NMC recognised mentor qualification
* Registered and up to date mentor
* Diploma or degree in healthcare
Skills & attributes
* Ability to work effectively within an integrated team
* Able to plan, deliver and evaluate programmes of care and promotion of health and wellbeing
* Ability to prioritise work and manage own work time effectively
* Able to communicate effectively
* Demonstrates the ability to teach others
