Job title: Team Leader
Responsibilities
* Lead in the person‑centred care planning process by implementing, monitoring, and undertaking monthly evaluations; complete care plan audits; develop action plans; and be responsible for completing action plans.
* Assist in the smooth transition of any discharges from the home, including documentation, medication, and keeping the resident's next of kin fully informed where appropriate.
* Monitor the well-being of each resident, be aware of changes impacting care and care planning, and ensure changing needs are met with records amended accordingly.
* Practice safe systems of work across tasks and moving and handling of residents, assessing risks and ensuring safety of residents, visitors, and staff.
Requirements
* NVQ 3 or equivalent in care for care homes.
* Two years' experience in a senior position with the resident group.
* Ability to communicate effectively verbally and in writing.
* Ability to lead staff and act responsibly in the absence of the manager.
