Job overview

We are recruiting a passionate and proactive Clinical and Operational Lead for a new Neighbourhood Proactive Care Service.

This is an exciting opportunity to play a key role in a new team delivering community-based, person-centred care for residents. T he focus will be on holistic assessments, care coordination, addressing health inequalities and delivering on the strategic ambition of a ‘ neighbourhood health service ’. This role will have both an operational management and patient facing element .

This service will see the shift from reactive care to proactive identification by using risk stratification searches within Primary Care records. The searches will identify a cohort of patients who would benefit from targeted i ntervention to improve outcomes, their overall experience and reduce the likelihood of further unplanned admissions. The service will operate alongside a number of other acute and community services, with the ability to step patients up in to appropriate services - including the highly successful virtual ward models in South and West Hertfordshire .

This role presents as an excellent opportunity to work in a topical area of service delivery within the NHS right now and will provide the opportunity to gai n service development skills .


Main duties of the job

You'll be responsible for operational delivery of the service, leading and managing a multi disciplinary team.

You will be working closely with a programme manager, testing new models of care, identifying interventions which make the most difference to this patient cohort, and evaluating the success of the service. Clinically, you will be from a nursing, physiotherapy, or occupational therapy background and will have appropriate clinical experience to be able to support a cohort of residents with moderate to severe frailty who have had 2 or more unplanned admissions within the last year.

Person specification


Education/Qualification

* Registered allied health care professional or registered nurse
* Degree/ masters level qualification relevant to community practice or Evidence of recent CPD
* Post-graduate management and/or leadership training
* Clinical supervisor training
* Specialist Community Practitioner/District Nursing qualification
* Non medical prescribing


Experience

* Significant post-qualification experience in a relevant clinical field
* Demonstrate broad experience post registration working in a variety of clinical settings.
* In depth experience of working within multi agency / multi-disciplinary team
* Experience of working with single assessment process
* Experience participating in a research or audit project and implementing full audit cycle
* Experience in mentorship role
* Experience of leading and managing a team
* Experience of managing a budget
* Experience of managing and investigating complaints
* Experience of managing and investigating serious incidents
* Experience of undertaking service analysis / deep dive reviews.


Skills and Knowledge

* Experience of implementing change management programmes e.g. service development
* Demonstrate advanced leadership & management skills
* Advanced computer literacy


Other

* Experience of electronic clinical systems
* UK Driving License and access to a car
* Experience of delivering a mobile working solution
