Overview
As a Health Care Assistant you will work alongside nurses, paramedics, physiotherapists, occupational therapists and technical instructors within the Welwyn and Hatfield Integrated Community Team. The role involves providing evidence‑based nursing care to patients in their own homes. Service operates 8am‑10pm, 365 days a year, with flexible working patterns and the possibility of full‑time or part‑time positions.
Responsibilities
* Provide assessment and care services to patients with nursing or health needs.
* Visit patients in their own homes and deliver care and support as defined in the Care Plan, including personal care, hygiene, uncomplicated nursing procedures, exercise, activity and treatment programmes prescribed by therapists.
* Support patients in a range of activities to promote independent living and monitor their well‑being.
* Assist with administration and clerical tasks associated with the care process, including record keeping and use of the computer.
Qualifications
* Full valid driving licence and access to a car for business purposes (unless a disability exists as defined by the Equality Act 2010).
* Excellent communication skills and a collaborative working style.
* Willingness to support people to improve their health and to develop professionally within a dynamic team.
