Overview
Locala is seeking a motivated Band 4 Assistant Practitioner to join the Community Respiratory Team in Huddersfield. The post holder will provide and support holistic care to chronic respiratory patients, working with a multidisciplinary team of nurses, physiotherapists, occupational therapists and exercise professionals in a variety of community settings. This is a 22.5 hours per week, permanent role with a salary band of £27,485 - £30,162.
Responsibilities
* Be part of the team delivering Pulmonary Rehabilitation – group sessions and home activity programs.
* Work closely with Occupational Therapists and Physiotherapists to undertake assessments, implement treatment plans and support self-management for patients and their carers.
* Provide holistic care including equipment provision, general rehabilitation, and develop skills in symptom management programmes.
* Assess, plan and implement treatment programmes for patients with respiratory conditions with support from clinical colleagues.
* Apply multi-disciplinary skills in community settings; support patients in adapting to change and self-management strategies.
* Undertake home visits as part of the wider team to deliver key elements of care (equipment provision, rehabilitation and self-management support) and contribute to service development and improvement.

Requirements / Qualifications
* Multidisciplinary skills and confidence working in community settings.
* Experience of working with respiratory patients is desirable but not essential; training and support will be provided.
* Ability to work with patients and their significant others to support personal change management strategies.
